How often should the slack adjusters be adjusted?

Unless you are operating an older vehicle, the slacks should be automatic adjusters. However they still need checked for proper operation on a regular basis. I would suggest checking them every time you service your vehicle. Federal law says they must maintain proper adjustment at all times, and be inspected for operation annually.
