What did evacuee tags in World War 2 say?

it had the child's information on it. including their full name the date of birth which then gave their age. Also it has the address written down and sometimes the school. It may have had the train station that they came from. It also gave extra information describing how they look and the behaviour and any medical information. On the back it sometimes has the child's number for example it may say, '4400'
