Most are done professionally. I believe that they have products off the self for it, but not sure how they do.

You can also try something called Tat B Gone to remove a tattoo. It actually fades it away slowly by breaking down the ink in your skin and you keep using it until your tattoo is faded away. It works well as I have tried it but keep in mind that it takes many applications over many months. You can buy it off their website or other website I've seen carrying it.
