The doctor in the civil war was facing hardship. They have no tools and medicine for the soldiers to be safe. Tools in the past was sharp. For example, if a person was shot in the leg, the doctors have to cut their leg in order to stop the bleeding. A bullet ball can hurt. They would not take it out of your body. A thousands of American died from diseases and too much blood.

Well plus they only had one painkiller alcohol. Which had adverse effects to it like being drunk disorientated and moody.

It is petty self explanatory.
