Get Sims 2 Body Shop for PC, choose a male, pick tight underwear, call it something upload it, go the Sims file, find 'Projects'. Choose what you called the project, choose the second image edit it, paint it all black. Save it, go to body shop, open the project again. Publish it to the game, play, if you chose it to be underwear, go to the underwear file then pick it, same with female.
