How do you get a lizard license?

First you must contact your state/province/country's Fish and Game, conservation center and possibly pay a fee. Sometimes you just apply for it, pay a fee and then you get your license in the mail.

Other times you have to take a test in order to receive a lisence. It all depends on where you live, and if the animal you are thinking about is even legal to have (even with a permit) in your area.
