First, read the Health Markets website (www.healthmarkets.com) and look at all the news articles posted about consumer-driven health insurance plans. A transcript of testimony to the US Congress is posted in which the CEO President testifies that healthcare is insane in the US when it comes to asking for the price for procedures (giving the example of buying a car and not being told the price until the mail comes after the buyer drives off with the car). One page on the website gives a map of the US, and shows which states have the coverage available. Unfortunately, the employees of the 3 insurance companies owned by Health Markets have not yet read the website, and in many cases they know less than a random consumer who reads the site. However, there are people who know about it, so be persistent and keep trying. It helps to use the terms from the website. One plan that has it is Mega Life and Health Insurance Care One Select. The basic idea is that the covered people can go online and see price and quality information for 20K procedures (tests, exams, operations) for 450K doctors and 150K facilities, including the contracted price, the maximum paid by the insurance, what education and certifications the doctor has, and--in some cases, perhaps--how many of that procedure that doctor has done and how many were successful.
