How do write letter for issuance of Sim card?

we have lost our mobile no ....................with sim card on dated ................

therefor you are reqested,please reissue the same no with new sim card. we are submitting the following documents required by you.

1. photocopy of telephone bill for id proof

2. one passport size photograph
