Different cooling, different materials, different ECU programming. The engine is cooled by sea water, additionally so too is the exhaust manifold, which is not seen on automotive engines.

Stainless steel water transfer pipes are used instead of cheaper rubber or pressed steel for risk of corrosion.

The engines are typically designed to run at permanently high revs depending on the marine application.
