Haha. When you find this answer, let me know. My boss does the same. I've resorted to reminding him of what time it is, how many pts he is behind and what he has coming up(appt load) also, if possible follow him/her into the room to assist w pt visit. I'm in an OB/GYN clinic .. He has to have us with him. So when I feel he's small talking, I start reaching for gloves. Or the door & clearly stop paying attention. On occasion, I walk out without him. Leaving him to talk nonsense (non medical) talk by himself. I've got stuff to do... He sees the queue & usually follows suit. When he throws a fit due to being behind, I remind him where he went wrong. And remind him that his pts still love him. And that we warn them of the delay before hand. Therefore he does not feel the need to compensate. By chatting. At times I present the problem & my assessment & plan. Many times he agrees and it makes the visit easier.
