How write an application for itikaf?

With deep reverence, it is stated that as you know holy month of Ramadan is going on and I have to go for performing Aitikaaf during this prestigious month and for this purpose I would require leave for Ten Days. The Reason is we have to sitting in an Aitikaaf. Kindly sanction the Leave from May 03, 2019 To May 13, 2019 We Shell remain obliged.
