Go to a professional to get it done. Visit them regularly. I recommend NOT to do it yourself. The reason it is falling out is because your hair is in bad condition. Before you get it done make sure that your hair is in good condtion. For a few weeks before hand use a conditioning treatment to get it into good condition. If it is the case that you are overbleaching your hair then you should not get it bleached for a while: give it a break.
