A telltale sign of being under the influence of heroin or any other opioid is pinpoint pupils (constricted/small). You also commonly see euphoria, decreased respiration, and slowed mental impairment.
A lot of the time too if they are really strung out they will have lesions on there face, lots of little sores or cuts.
