If you find a young wild European Hedgehog that appears in distress (sick or injured), then you should obtain veterinary care for it. Contact your local vet (some treat wildlife casualties for free) or wildlife rescue centre. Many hedgehogs are born late in the year and fail to put on sufficient weight to survive hibernation. If you find a small hedgehog during the autumn or winter then contact your local wildlife rescue centre. There are some steps you can take in the interim to care for the animal, however - see the related link.
If the hedgehog is out during the summer, in the evening, and looks healthy then leave it alone.
