I think that the best way to answer this is to say that most shipwrecks have been caused by poor situational awareness and poor navigation.

For example, when the ship's position is thought to be one place, but it is actually another, and no one on the ship notices the error, the ship can run aground...things like that.

With the advent of GPS and better charts, shipwrecks have decreased in frequency, but...nothing out there replaces and experienced person in charge of the ship or boat, that knows what to do.
