What would be an egregious error in an English book?

How about misnumbering the pages, or getting the chapters out of order? "Egregious" means "out of the flock" or more loosely, "outstanding in a bad way." An egregious error presumably distinguishes itself from more pedestrian errors by its exceptional magnitude, carelessness or consequences.
