But you cannot obtain a copy of medical records over the phone. Either you go there and request personally or you may write a letter. If you decide to go there, go to room 101.

When you write a letter make sure you do the following:

Attention to: Medical Records

Address to: 1000 W. Carson St., Torrance, Ca. 90509

Write your name and DOB. Make sure you write all information of what you are requesting. e.g. diagnosis, lab results, xray results etc...

Write also the name and address of your doctor so Harbor can mail your records to your doctor.

It usually takes 15 working days...
