You need to get a police report that you have lost your ID card

Then take your passport to the Pakistan embassy they will take your biometrics and will issue you a new over seas Pakistani card called NICOP. I think no appointment is necessary as I live In the UK and I have been to all this recently

The new card could be collected in 15 days normally
