there are many ways that structual steel can lead to failure. the way it comes out of the steel mills during pouring and casting, tensile strength in bolts, wrong sized steel use due to miss calculated arcit. and structual design not being over-rated, missed steps in erection in structure or rebar such as cad-welding versus tie-wire. unsat. weld missed by the inspectors. there is so many reasons why a structual steel building has failures. my experience is about 75% of the time is carelessness, to big of a rush by everyone and the sub-contractors know it but higher above play it off.
