What does it mean when a disease has been eradicated?

This means that the organism that causes the disease has become extinct (at least "in the wild", there may still be samples in national labs kept for scientific study).

We have only done this to one disease: smallpox (the US and Russia maintain the only two lab samples of the virus in cryogenic storage).
