It is not necessary to remove all your clothes in front of anyone especially when a child at school. Physical exams should be properly supervised by a medical doctor in the presence of the child's parent/s, not conducted by a school nurse..

If you are unhappy about any aspect of these type of medicals then you do not need to forgo them.
