There is no "code" to unlock Five. In order to unlock five, you must first complete the Campaign on any difficulty. However, if you break away from the chair at the main menu (Look Down and press both triggers at the same time) and go to the Computer at the back of the room and type in "DOA", (which by the way stands for "Dead on Arrival") you will unlock the Game Type "Dead Ops Arcade".
