Traditionally middle names tend to be lineage-related. For instance:

The name of an honored relative or someone else held in high esteem.

The grandmother's or mother's maiden name.

Or

The first or middle name of one of his or her parents.

Middle names can also be derived from the parents religion, such as Patron Saint.
