The blanks that appear on Form 4868 Extension:

1. Full name and address

2. Your social security number

3. Spouse's social security (if married)

4. Estimate of TOTAL TAX liability for 2011.

5. TOTAL of all payments toward taxes for 2011.

6. Balance due if #4 above is larger than 5, you should try to pay this with your extension to avoid penalties. If you don't have the money, atleast you avoid paying LATE FILING penalties, but you will begin to accrue some interest on the amount you owe (this depends on your last year's tax liability and the date you finally pay).

If #5 if larger than 4, you expect a refund and leave box 6 blank.

I did an online extension (free) and it also asked me my filing status, but that doesn't appear on the IRS FORM 4868.

I have to say one thing, the problem is that many people file extensions because they don't know the answers to #4 and #5 above! I've never heard of repurcussions to entering this information to the best of your knowledge.. I've even sent one in blank before and as long as you file timely, you're probably okay!
