Is a new substance formed when two chemicals are mixed?

If they're mixed, but remain separate (as in a solution) then the answer is no. But if the two chemicals undergo a chemical reaction with each other then at least one new substance will be formed; if there are by-products from the reaction, then more than one new substance will be produced.
