You are looking for the propor wording to say you are forwarding your name for the position of counselor?

I am applying for the position of counselor you (name the firm, hospital or whatever the place is). Finish off the letter and sign it then attach your Resume. When you do this letter you are selling yourself so be sure you tell them that you get along well with coworkers, that you are Excellent in the line of work you are in and feel you would be an asset to their firm (or whatever the title is.)
