All health care settings use ICD-10-CM to report A. equipment B. diagnoses C. procedures or D. services?

An internet search of "ICD-10-CM" found a pdf titled "CD-10-CM Official Guidelines for Coding and Reporting FY 2017" that states in the first paragraph "The ICD-10-CM is a morbidity classification published by the United States for classifying diagnoses and reason for visits in all health care settings." Seems to me the choice would be "diagnoses".
