How do you remove 1949 Cadillac door handles?

Wow, a 49 caddy!!!!! The majority of door handles from this era were bolted on from inside the door. The door liner must be removed to access the bolts. I am not 100% sure about your 1949 Cadillac, but suspect it is the same.
