How can a 12 year old pierce her lip whit out hurting?

That would depend on each individuals personal pain threshold.

But it would hurt anyone more if the piercer had no proper training or equipment (like if someone pierced their own lip with a sewing needle)

Either way it would hurt, no matter what, as it is jabbing a needle through a body part.
