How do you write exam leave application to an office?

Respected Sir,

I would like to seek your permission to appear for my M.C.A. Final examination (5-Papers). This is scheduled to be held 6, 10, 11, 15 &31st of Dec. I want leave on 5th, 6th (Paper), 10th (Paper), 11th (Paper), 14, 15th (Paper) and 31st (Paper) of Dec. During this period, I'll not be able to attend the School as my exam dates.
