In block letter format, start with the addressee. The first paragraph will state the time/date of the incident and the type of violation if completely defined by company code of conduct. The next paragraph will request a written response to the accused incident with a given deadline, typically less than 48 hours. The last paragraph will offer a meeting time to discuss the accuser's response and will give a due course of disciplinary action, in the cause that the explanation is unsatisfactory. The closing will be in block format, given with a signature.
