If there are signs of infection -- leave it out. Let the hole heal, and try again. Likewise, if the hole or channel has been torn, either let it heal or get a medical opinion about whether it needs sutures or not. If there are no signs of infection, and if the channel or hole is intact. Then youi need to flush the channel with hydrogen peroxide (3%), and soak the stud or ring in alcohol for 20-30 minutes. Then let it air dry before re-inserting. If it falls out you probably need a larger gauge jewelry item. The exception to this is why its falling out. If it got tugged on and the piercing itself tore then you may want to get it redone or simply let it heal. Also if it had a "screw-on-ball" back piece of jewelry and that's missing or something is wrong with the piece of jewelry, then it may simply need to be replaced. If you are concerned about the piercing site, and are unsure if it is infected or not, I'd recommend that you see your Dr. or seek Medical help to determine if you need further medical attention.
