Why do people use the term no excuses?

People use the term "no excuses" as an expression that one should be held responsible for their actions. At some point, the excuses do not matter, and the only thing that matters is results. A teacher might say, " no excuses" for turning in home-work late, because even if unknown circumstances present themselves, one should complete their homework as soon as possible.
