What is the matter and form of the Sacrament of Holy Orders?

The matter of the sacrament is the laying on of the bishop's hands with the consecratory prayer (cf. Catechism of the Catholic Church, no. 1538).

The form of the sacrament is the bishop's "specific consecratory prayer asking God for the outpouring of the Holy Spirit and his gifts proper to the ministry to which the candidate is being ordained" (CCC, no. 1573).
