The success of the no child left behind program has been hampered by?

The fact that promoting children into the next grade when they aren't ready for it just serves to pass on their troubles to the next grade where the demands on the student are all the more complicated. (i.e. if they could not pass grade 3 math, how in #@*!*#@ are they supposed to pass grade 4 math!?!?)
