How do you get a replacement of fireguard license?

Lost Certificates of Fitness may only be replaced in person at a cost of $5 per duplicate. To replace a Certificate of Fitness, the Certificate holder should provide Government Issued identification (such as a Driver's License or Passport) with his / her full name, social security number, and home address. The name of the Certificate of Fitness to be renewed and payment are also required.
