Is a thank you required for a birthday card?

it is not required, but it is very nice. think of it this way: you send someone a very nice gift because you care and you believe it will help. after you give it, you receive a card saying how much it has helped and how thankful the person receiving it was. would't it make you feel better and want to give more?
