The helicopter crash which killed SRV was never definitively ruled on as pilot error, but that does seem to be the consensus.

The helicopter took off into foggy conditions and crashed into a ski resort less than a mile from takeoff. It is likely that the pilot became disoriented in the foggy conditions and did not climb to a high enough altitude to clear the mountainous terrain.
