ANYONE - degreed or not can affex Dr. before their name (although it may be deceptive, unethical, and dishonest). Anyone with a doctoral degree such as a Ph.D. or Ed.D can legitimately put Dr. in front of their name. The potential problem is how they represent themselves. If they present themselves as physicians, they can get in trouble for practicing medicine without a license. Since most jurisdictions do not issue licenses for most types of alternative medicine, they will not be able to legally present themselves as "doctors" in the medical sense. Note that there are some fields that might be considered "alternative medicine" where they practitioners DO get licensed - such as Chiropractics and Osteopathic medicine.
