First and foremost you must see a doctor. You need to have a professional give you a physical to determine a safe and achieveable workout program for you to begin. It is also a great idea to see a nutritionist to give you a healthy template for your new diet routine.
Once those steps are done it comes down to discipline. Follow the workout guidelines your doctor gave you (or create your own with the boundaries the doctor has given you) and be sure to stick closely to the diet that your nutritionist has discovered.
The trickiest thing is that true and successful healthy living takes years. Beware of any product which boasts "lose weight in one month" because they are going to throw your body into a cycle of losing weight quickly, then scrambling to put it back on, then losing it again, then putting it back on, and etc...
Just remember, a six pack is a lifestyle, not a target. Good luck!
