The typical contents of a Project Management Plan (PMP) are:

  • Project Overview/Background
  • Project Execution Strategy
  • Scope of Work
  • High-Level Project Schedule
  • Milestones/Deliverables
  • Project Organisation
  • Monitoring and Control Methods
  • Risk and Opportunity Management Plan
  • Project Budget
  • Change Control Procedures
  • Project Quality Plan
  • Communications/Reporting Plan
  • Stakeholder Management Plan
  • Resource Management Plan
  • Procurement Plan
  • Health & Safety Management Plan
  • Information Management Plan

Don't forget the reference to the Business Case, and don't forget to go through the proper document management process (document title, author, date, version number, issue date etc). The sections can make reference to other plans in other documents -0 it doesn't have to be all in one document.
