I believe it depends on the ring. Usually the lesser expensive rings come in the little Tiffany pouch. I have a couple of sterling silver Elsa Peretti stacking rings, one has an emerald in it, and the other one even has a diamond. They both came in the pouch, but they were sterling silver and had very, very tiny stones.

At a certain price point - you will get a black velvet box. Definitely, for engagement rings and wedding bands. My friends with engagement rings from Tiffany's all have black velvet boxes. They probably also throw in the fancy velvet box for gold, platinum or any ring with numerous larger gemstones as well.

That being said, if you purchase at the store - you can always ask for the box you would prefer. My friend just bought a Tiffany key necklace and they put it in a black velvet box. The necklace was only around $250, and definitely should have come in a pouch instead. However, he just bought an engagement ring for his girlfriend a couple of months beforehand. He went back to the same sales person that sold him the engagement ring and presto bingo - fancy box upgrade for the silver necklace! If you develop a relationship with a sales person or ask nicely - I'm sure they will accommodate you. Either way, it seems the packaging to some point is at the sales person's discretion.

Online or through the catalog though, you should assume that cheaper rings will come in the pouch and that you won't get a nicer box until you start spending a lot of money.

Regardless of whether the ring itself comes in pouch or a box - the pouch or the box will then be placed inside the famous Tiffany Blue Box with white ribbon. Every purchase from Tiffany & Co., no matter how small, is afforded the same little blue box treatment.
