A social worker must report it to their line manager or in-charge immediately and corrective measures should be implemented as soon as practically possible, to avoid harm or injury. If it involves poor working practise staff must be trained, if they are resource difficulties the employer must provide the adequate resources. And risk assessments must be carried out and correct the anomalies.
