How do you send a note announcing the death of a coworker grandmother?

I just had to do this today, unfortunately. This is what I wrote:

I am saddened to report the passing of EMPLOYEE'S grandmother, who passed DAY or DATE. A card is being routed through the OFFICE/BRANCH/BUREAU/UNIT along with an envelope to take a collection for a GIFT/PLANT for EMPLOYEE. Once arrangements of a memorial service have been made, the information will be shared with the OFFICE/BRANCH/BUREAU/UNIT. Please keep EMPLOYEE and HIS/HER family in your thoughts during this difficult time.
