It doesn't, it's mainly for show.

Back in the days it was used, not to make the tequila stronger, but to prove how strong it was.

If you dropped a worm in weak tequila it'd eventually dissolve, but if the alcohol content was high enough the tequila would act as a preservative and the worm would remain intact.
