Basically, it has to be served by flights to other countries.

It therefore has to have Customs and Immigration facilities to deal with international arrivals. It also has to be sufficiently important to attract international airlines. This is why in the US, where most flights are domestic, many international airports wear the "international" badge with pride. In Europe, most flights are international so the distinction is less common.
