How do you plot visual case analysis for syphilis?

Visual case analysis (VCA) for syphilis requires a chart, called the CDC (73.10) Syphilis Epidemilogic Analysis Chart, (on which to plot the original patient (who is infected with syphilis) and the sex partners. Serology information and the patients' signs and symptons (if any) are needed for VCA.
