Can a cat have only one baby?

Yes. Sometimes labor can take a long, long time. But if 12 hours have passed and she's made no changes, I might suggest a vet. Simply because it sounds like those kitties will never come out otherwise. If they're stuck, it might be fatal to kittens and mothers.

Alternatively, she may just have been pregnant with one kitten.
