Sample of excuse letter in work?

When an employee/future employee misses work/interview, a leave of absence letter to the boss is sometimes required. This should include the normal parts of a letter, such as the heading, salutation, body, closing, and signature. The letter should also include the requested day off work and the reason it is being requested.
