Why did the biomedical revolution of the twentieth century cause population growth?

1. more children are born (to what were infertile people)
2. more children survive into adulthood
3. less adults die of medical conditions so more breeding happens
4. adults live significantly longer.

There is also growth caused by some people having more than 2 children and not using another medical aid called contraception.
